- Are living with a life-limiting condition and wish to remain at home
- Need comfort-focused support alongside medical input (GP, district nurses, palliative teams)
- Have increasing frailty, reduced mobility, fatigue, or personal care needs
- Require dignified support with daily routines during a sensitive stage of life
- Need consistent carers who understand pacing, comfort, and emotional reassurance

- Need reassurance, guidance, and practical support during an emotionally difficult time
- Want a calm, accountable provider who communicates clearly
- Are exhausted, anxious, or afraid of leaving their loved one unsupported
Risks & Challenges We Actively Manage
Palliative care becomes unsafe when risks are not recognised early. We actively manage:
- Pain or symptom distress escalating without support routines
- Fatigue, weakness, and falls risk
- Skin integrity risks due to reduced movement and frailty
- Poor nutrition/hydration due to low appetite or swallowing difficulties
- Medication complexity and timing risks (as agreed and assessed)
- Anxiety, agitation, confusion, or emotional distress
- Family overwhelm, burnout, and guilt
- Communication gaps between professionals, carers, and family

- Medication prompts/assistance/administration in line with agreed plan
- Accurate recording and escalation if concerns arise (symptoms, side effects, changes)
- Liaison with family/GP/district nurses when appropriate for continuity

- Calm presence, reassurance, and respectful interaction
- Reducing fear and distress through stability and familiarity

Domestic Support & Practical Stability
- Light daily-living support to keep the environment settled and safe
- Support with meals/hydration routines where appropriate
- Helping maintain a calm home atmosphere (predictable routines, gentle structure)
Nurse-Led Oversight & Governance
As a nurse-led, CQC-registered provider, this pathway is governed to ensure safety and accountability:
- Comfort-focused risk assessment and care planning
- Monitoring for deterioration and early escalation where needed
- Safe medication support processes (within agreed scope)
- Safeguarding awareness and documentation standards
- Supervision and guidance to carers so care is consistent, not improvised

Family Reassurance & Escalation
Families are supported through:
- Clear communication ā without panic and without surprises
- Early escalation if comfort needs change or risks increase
- Practical guidance on what support can be put in place
- A clear point of accountability for decisions and concerns
